Request your pet’s medication refills for pick up at our office, here! Just fill out the form below and you will be notified by text when it is ready for pick up, if your pet is eligible for refills. All pets must have a current exam on file within the past 12 months and be current on blood work for certain medications.
All medication requests require a doctor’s approval, please allow for a full business day for your request to be processed. If you need immediate assistance or have questions about your pet’s medication, please contact us at (408)996-1155.
Please note: This refill request form is for medication pick up at West Valley Pet Clinic. If you would like to have your order delivered directly to you and take advantage of promotional pricing that might be available, please use our online pharmacy instead.
